Capacity Assessments
We provide professional, in-person capacity assessments completed by Designated Capacity Assessors trained through the Office of the Public Guardian and Trustee of Alberta.
Assessments are available at our Leduc, AB clinic or in your home, care facility, or community setting anywhere in Alberta—making this service accessible and convenient.

Steps To Obtain a Capacity Assessment
Before Your Assessment
To proceed, the following is required:

A free initial consult with the applicant (by phone or video)

A recent medical assessment (within the past 3 months) completed by a physician
This ensures the individual being assessed is not experiencing a temporary or reversible medical condition that could affect decision-making.
If the client does not have a family doctor, a walk-in clinic or any physician assessment is acceptable that confirms there are no temporary or reversible medical conditions present. Our clinic will require a letter from the physician or a copy of the health records to legally proceed with a capacity assessment
Our clinic does not complete capacity assessments for individuals that have Personal Directives in place, as they require a Physician or Psychologist to complete a portion of this assessment.

Assessments typically take 1–3 hours to complete
What's Included

A detailed written capacity assessment report

A copy of the report for submission to the courts or the Office of the Public Guardian and Trustee (OPGT).
Please note: Filing paperwork is the responsibility of the applicant. Capacity Assessments must be filed within 6 months of completion with the courts or OPGT.

Mobile assessments are ideal for clients with mobility, physical, or mental health challenges, and can provide a more accurate picture of daily functioning.
Travel and mileage fees apply for mobile services and will be discussed in advance.

Click the, “Adult Guardianship and Trusteeship,” section to expand

Click the, “Application Bundle,” to obtain all the forms the applicant who is applying to be the Guardian or Trustee will have to complete and submit with the Capacity Assessment Report (Form 4) you will be provided by the Designated Capacity Assessor.
