Requirements Prior to a Capacity Assessment
A consult by video or phone must be completed with the capacity assessment applicant to determine a capacity assessment is appropriate in your unique situation. A capacity assessor is legally required to obtain a copy of the client's health records to confirm a medical assessment has been completed by a physician within the three month period immediately preceding the capacity assessment. This is to ensure the adult being assessed is not suffering from a reversible, temporary medical condition that would be likely to have a significant impact on the adult's capacity to make a decision about a personal matter or a financial matter.
Without a medical assessment (check up) being completed in the past three months on the client, a capacity assessment can not legally be completed. If you don’t have access to a family physician a walk in clinic physician or hospital assessment would be sufficient as long as the capacity assessor can obtain a copy of the health records. Fees for Service
$500 - Fee for a capacity assessment in one the three areas of:

Co-decision-making​

Guardianship

Trusteeship

$700 - Fee for combined Guardianship and Trusteeship Capacity Assessment

Capacity assessments usually take 1-3 hours depending on each individual client. Assessments are not rushed to ensure a thorough assessment. Our team takes these assessments seriously and understands the results of the assessment can be life changing for clients and need to be handled with care and professionalism.
The listed service rates include:

Initial consult with the capacity assessment applicant by phone or video call

Reviewing and obtaining health records from the client’s health care team

Formal capacity assessment with the client to assess for guardianship and/or trusteeship

Completion of a capacity assessment report with recommendations if the client has capacity or lacks capacity in certain domain areas. Determine if formal guardianship or trusteeship are indicated or if possibly co-decision making may be appropriate.

​A copy of the capacity assessment is provided for the applicant to submit (to the courts or the Office of the Public Guardian and Trustee Office of Alberta).
A capacity assessor does not file the assessment paper work, it is the applicants responsibility to file the paper work accordingly.

If you are requesting a capacity assessment to be completed at a location other than the clinics Leduc, AB office, additional fees for time and mileage will apply. These additional fees will be discussed prior to completing services and are fees in addition to the fees listed above.

Locations Capacity Assessments Can Be Completed
Our team offers the option of completing a capacity assessment at our Leduc Clinic location which is located on the main floor and has wheelchair accessible ramp. Or there is the option of our team going mobile within Alberta to your home, care facility or other location that is most comfortable for the client. If the client has mobility, physical health or mental health barriers to being seen at our clinic location our team can come to you. Our team wants to ensure clients have access to the assessments they require in a community based setting. Assessing a client in their home environment can be helpful in getting a clear picture of their overall functioning, and their ability to navigate their needs day to day.
